"Where There's A Will,  There's A Way" Grant Program

In 2010 families with special needs loved ones will be eligible for greatly reduced, grant subsidized wills, testamentary special needs trusts and end of life documents sponsored in part by grants received from the Preventive Family Support Services. To qualify families must follow the following grant guidelines:

• Perform a phone intake by calling our office at (954) 806 7589
• Provide proof of documented diagnosis
• If legally married, both parents must attend an orientation workshop and grant interview
• Accept placement in grant program within 30 days of receiving grant award letter
• Provide full disclosure of requested legal, financial, personal information and documentation
• Participate in round table discussions with potential guardians, trustees, conservators and other key legally named individuals
